Contact your insurance company to get information regarding your benefits, deductible, co-pay, etc.
No Show/Cancellation Policy
I understand that a scheduled appointment is an hour that is set aside for me and/or my child and that by not showing up or canceling late, I am not allowing that appointment time to potentially be scheduled for another client. I understand that I am expected to email/call/text Safe Haven Counseling 24 hours or more prior to my scheduled appointment time and that failing to do so will mean I am billed 50% of appointment cost directly (as this is not billable to insurance). I also understand that I will be asked to provide a credit/debit card on file for this purpose.
